Accepted Insurance Plans

In addition to the plans listed above, we accept any other PPO plan. However, if we are not in-network with your PPO, you may be responsible for a larger portion of the cost compared to seeing an in-network physician.

Insurance Plans That Do Not Cover Visits

Unfortunately, we are unable to bill HMOs that are out of network. If we are not in-network, your insurance company will not cover the cost of your visit. To help make your visit more affordable, we offer a self-pay discount for our patients.

Questions About Your Coverage

For specific questions about your insurance coverage, we recommend contacting your insurance company directly. You can reach their customer service department by calling the number on your insurance card. They can assist you in locating an in-network provider and provide plan-specific information.

Contacting Your Insurance Provider

Each insurance plan is unique, and only your insurance company can provide the most accurate information about your coverage. You can contact them by calling the customer service number on the back of your insurance card. If you've misplaced your card, your Human Resources department or school's health services can provide you with the contact information.
