Caremont Health is a Direct Primary Care facility dedicated to providing compassionate, comprehensive, and patient-centered healthcare. We offer a wide range of services including primary and preventative family medicine, acute care, chronic care, and more.Our members enjoy virtually unlimited access to in-person and telehealth appointments with no co-pays. At Caremont Health, we ensure you receive the care you need, whenever you need it.

How It Works

First Step

Enroll online through our website. After enrollment, check your email for the "Next Steps" email to create your username and password for your patient portal.

Second Step

Log into the Patient Portal to complete the "Next Steps" process. This includes uploading your required IDs and filling out your intake forms.

Final Step

Schedule your first visit through the "Appointments" tab in the Patient Portal.
