Enroll online through our website. After enrollment, check your email for the "Next Steps" email to create your username and password for your patient portal.
Log into the Patient Portal to complete the "Next Steps" process. This includes uploading your required IDs and filling out your intake forms.
Schedule your first visit through the "Appointments" tab in the Patient Portal.
